This program is intended for students entering 6th-8th grade. Program registration is limited to 15 participants. Registration deadline is June 1, 2014. Upon successful registration, participant will receive email confirmation with further details about program.
$100 if registered by May 26, 2014
$110 on May 27, 2014 or after

- Flight in K-State aircraft with certified flight instructor*

*Participants not wishing to participate in flight should contact Continuing Education prior to registering.

- Signed copy of Participation Waiver submitted to the Professional Education & Outreach office.
- Copy of medical card (front and back) submitted to the Professional Education & Outreach office
- Signed copy of Department of Aviation agreement submitted to the Professional Education & Outreach office.
Participants and their parent/guardian should also review and understand the Discover Program participant expectations.
Registration for this event gives consent for photographs, publicity and inclusion on the participant list unless the Professional Education & Outreach office is notified in writing at email@example.com prior to the start date.
A group photo will be taken on the first day of the program. Additional photos and recordings will be taken at the discretion of program coordinators.
Substitutions will be considered on a case-by-case basis. Contact the Salina Professional Education & Outreach office to request a substitution.
Payment is required by the event date. We accept VISA, MasterCard and Discover credit cards or you can pay by check. To make a payment by credit card call 785-826-2633 or toll free 855-552-0079. Checks should be made to K-State Salina Professional Education & Outreach and mailed to K-State Salina Professional Education & Outreach, 2310 Centennial Road, Salina KS 67401.

CANCELLATIONS AND REFUNDS
Cancellations are the individual’s responsibility. Cancellations must be received by K-State Salina Professional Education & Outreach by 5:00 PM CST on the registration deadline date. Refunds will only be honored if the cancellation is received by the registration deadline. A $20 administrative fee will be deducted from the refund.
Cancellations received after the deadline are not eligible for a refund. Registrants who do not cancel their registration are responsible for the registration fee even if they do not attend and have not paid the fee.
K-State Salina Professional Education & Outreach may cancel or postpone any course or activity because of insufficient enrollment or other unforeseen circumstances. If the event is canceled or postponed, the Professional Education & Outreach office will refund registration fees, but cannot be held responsible for costs, charges, or expenses, including cancellation/change charges assessed by airlines or travel agencies.
A participant with a disability who needs accommodations or has special dietary requirements should indicate services needed at the time of registration.
