"Radical Integration: Redefining Whole-Person Care Across Clinical, Case Management, and Community Teams"

Join K-State Salina Social Work in empowering interdisciplinary teams! We’re looking for interactive workshop leaders and speakers ready to deliver practical, high-impact strategies for clinicians, case managers, school personnel, and community health workers. If you have actionable tools or workflows connecting behavioral health, frontline care, and social determinants of health solutions, submit your proposal today.

For best consideration, we encourage you to submit your application by December 4, 2026. The final application deadline is December 21, 2026. We will communicate selection status to all applicants.

Conference Overview

• Date of Conference: April 22, 2027
• Event Format: Hybrid (In-Person and Live Interactive Webinar)
• Presentation Length: 60-minute, 90-minute, or 3-hour intensive sessions will be considered
• CE Qualifications: All sessions must meet state licensing board criteria for continuing education units (CEUs) for social work.

Possible Focus Areas & Topics

Presentations could relate to one or more of the following suggested topics:

• Health Literacy: Common chronic & co-occurring medical conditions that social workers should understand, including physiology, symptoms, common medications, and behavioral impacts of high-prevalence chronic conditions.
• Teamwork and Handing Off Care: How doctors, therapists, substance abuse counselors, and case managers can work together smoothly. Include best practices for "warm handoffs”, introducing a patient to a behavioral health or social helper during a medical visit.
• Advanced Case Management: Strategies for helping patients with multiple overlapping needs. Strategies for coordinating care when a patient transitions out of the hospital, navigating complex systems, or partnering with local legal aid services.
• Non-Clinical and Community Roles: How to expand care using the unique strengths of community health workers, peer support specialists, and medical assistants. Utilize frontline staff to build trust and bridge cultural gaps between providers and communities served.
• Screening and Brief Intervention: Real-world ways to use screening tools (like PRAPARE, SBIRT, Etc.) to identify behavioral health and social needs and offer brief intervention strategies. Without overwhelming your staff.
• Technology: Important considerations in using telehealth, healthcare apps, and artificial intelligence in behavioral health practice.
• Connecting Local Resources: How to build strong networks with local food banks, housing programs, and utilities. How to create workflows that guarantee a "closed loop” to confirm the patient actually received the community help.
